The 2nd annual Chronic Disease Management Conference will return on 22-23rd November 2022. Co-located with the National Health Innovation Precinct Summit, Royal Randwick.
There is a need for significant and long-lasting change in the way chronic disease is recognised, treated, and managed in Australia. Around 47% of Australians now live with chronic health conditions and 37% of all hospital admissions are related to chronic health conditions.
This event is a critical platform for national discussion, thought leadership, a showcase of best practice and networking to help drive real change across Australia. Bringing together high-level executives, healthcare providers, policy makers, chronic disease specialists health service leaders and administrators, government representatives from both state and federal governments.
