New for 2024 – The Pharmacy in Aged Care Forum

The Royal Commission into Aged Care Quality and Safety identified medication management and safety in residential aged care homes as an essential area for improvement.

In response, we’re delighted to bring this multidisciplinary forum focused on improving medication management in aged care to Melbourne in 2024. Running concurrently with the 1st day of the National Dementia Conference at the Crown Promenade in Melbourne, this forum will provide an opportunity for discussion, sharing of best practice and networking between stakeholders.

Some of the themes to be explored will be around improving processes and systems to ensure medication management, using quality indicators, maximising the role of medication advisory committees, medication reviews, new guidelines and addressing challenges in transitions of care.

Medication administration is one of the most time-consuming activities undertaken in RACFs. In this presentation Professor Sluggett will discuss their implementation of a medication simplification service by pharmacists in RACFs using a 5-step validated tool which leads to more streamlined medication use and reduced burden for residents and providers. They found that implementing this service can lead to two thirds of residents taking their medications in a simpler way and it enables staff to shift a meaningful amount of time (~0.5 FTE/100 residents) away from medication rounds to other care activities to improve resident care and wellbeing.
